Agents of Change

The Abundance Agents of Change Challenge Grants charge Harvard students, in partnership with local community health centers, to create and pursue innovations in primary health care delivery. The innovations must be designed for populations served by community health centers. The program connects HMS students, community health clinics, physicians-in-training from Harvard-affiliated hospitals, and members of the Harvard University graduate schools to work toward a shared mission of improving primary care for the underserved.

Agents of Change 2017 Cohort
We are excited to announce the three teams that were chosen for the Agents of Change cohort running from November 2016 – February 2018.  

Interventions in Family Oriented Optimal Diet for Development (iFOODD)
iFOOD, in partnership with The Dimock Center, seeks to develop a family-focused nutrition curriculum to complement the multidisciplinary home-based visits of the Dimock Early Intervention (EI) team for children with developmental delay. They plan to embed the program in the extracurricular infrastructure of Harvard Medical School and the Harvard School of Dental Medicine, and will develop a “playbook” to enable the program to scale to other medical and dental schools across the country. Social Determinants of Health Collaborative
The Social Determinants team, in partnership with the Boston Health Care for the Homeless Program (BHCHP), aims to create tools and training programs that enable the integration of care and services for homeless persons to improve their health outcomes and decrease avoidable emergency department and hospital usage. BHCHP is one of nine service providers taking part in a two-year pilot of the Health Policy Commission Consortium to integrate care of homeless persons; the Social Determinants team will be a critical contributor to the Consortium’s work. The PERSONAL Project

The PERSONAL project team, in partnership with East Boston Neighborhood Health Center (EBNHC), is developing a motivational messaging platform that utilizes an adaptive learning algorithm to personalize the messages to the unique personality, preferences, and needs of each patient. Read more
