Primary care leaders and teams must navigate financial constrictions, low workplace morale, restrictive payment models, rising health care costs, increasingly aging populations, the integration of behavioral health, and achieving patient satisfaction. Through this interactive, case-based, executive education program, you will learn how two leading health care organizations have implemented successful strategies for managing their unique patient populations.

For example, the Camden Coalition’s Care Management model addresses health care, housing, and other social needs in an integrated fashion – which could be helpful for development of new strategic approaches in your organization. The Kaiser Permanente Complete Care model addresses how quality improvement and systems thinking were integrated into the new initiative, Depression Care Management, with successes and challenges in consistent implementation across a national organization.”


The Center Announces New Case Series

The Harvard Medical School Center for Primary Care announced today a new case series that will support the Center’s leadership development curriculum and programs for front line primary-care leaders, including physicians, other healthcare workers, and staff. The first two cases, currently available through the Case Centre, profile the work of Southcentral Foundation, an Alaska Native health care organization working with the Native Community to achieve physical, mental, emotional, and spiritual wellness.
