Systems Transformation at the HMS Center for Primary Care
We oversee a wide range and growing portfolio of programs dedicated to transforming primary care via practice improvement efforts and team-based care.
While the majority of the Systems Transformation team’s programs are dedicated to facilitating collaboratives among the Harvard medical community and in the greater Boston area, new grant-based projects are extending the team’s reach internationally.
